At Home Health CC Skilled nursing is a Medicare-covered service and is health care that is provided when skilled nursing services are needed like: wound care, intravenous (IV) therapy injections, post-surgical care, medication management, and monitoring of vital signs and medical equipment. Our skilled nurses are needed to manage, observe, and evaluate the care an individual receives and in the comfort of their own home.
Our main goal for our skilled nursing patients is to help the individual repair or maintain their independence and level of functioning while remaining in their home. Our skilled nurses do not perform custodial duties.
Our nurses serve individuals who
· Who have experienced a decline in condition
· Need assistance with activities of daily living
· Are sick with multiple co-morbidities
· May or may not have a disability
· Recovering from surgery
· May or may not have assistance with recovery care needs
In order to be referred to a Home Health CC in-home skilled nurse you have to meet these criteria:
1.) Be released from the hospital
2.) Recovering from surgery, illness/chronic condition
3.) Unable to care for themselves
The Home Health CC nurses start with an assessment of the patient focusing on their immediate health care needs. They collect and analyze data, identifies the clients learning needs and rehabilitation potential, diagnose and plan of care specific to the client’s health care needs. They provide ongoing communication with the client, family, and healthcare team to ensure the provision of optimal client care.
